Important information on updating immunization records

State law requires that parents submit an updated immunization record for students in kindergarten, Grade 7 and Grade 12.

Parents of students in kindergarten and grades 7 and 12: Please send your child’s immunization records to their school nurse or email the school nurse directly.

For all parents: As you anticipate preparing your children for the return to school, be aware that because of COVID-19, there have been some delays in well-child check-ups. Therefore, it’s important to schedule your child’s check-up/immunization appointments now.
