- OHSAA Pre-Participation Physical Evaluation needs to be turned into your schools Athletic Director before the first practice or conditioning date. This needs to be completed by a Medical Doctor. If you need a blank physical form, you can find them in the school office or on the forms page on this website.
- Final Forms Account - Final Forms need to be completed in full by the first practice. This includes parent signatures and student signatures. Instructions for student, and parents, on Final Forms have been passed out separately.
